Able to perform all aspects of patient care from pre, intra and post-procedure, from admission through discharge. During the admission process is able to focus on assessing, identifying, and solving nursing care and pre-procedure problems. The nurse is responsible for starting and monitoring IVs, administering pre-procedure medications, performing pre-procedure instruction and explanation of the patient's rights and procedures. Able to care for patients in recovery phases I and II. During patient recovery is responsible for monitoring vital signs, providing pain management, providing patient and family education, and ensuring the safety of the patient until discharge into the vehicle of a responsible driver. DUTIES AND RESPONSIBILITIES:Registered Nurse-3 (Sedation RN)Pre-Procedure:1. Assess physical and emotional needs of patients and implements appropriate nursing interventions. Completes nursing pre-assessment record including confirmation of the following:a. Allergiesb. Home Medicationsc. Dentures, Contact lenses or Glassesd. Pertinent medical conditions and medical historye. Confirms procedure and/or operative sitef. Obtains signature on procedure consent form per doctor's order.g. Confirms responsible party and transportation home per facility policy.h. Obtains any relevant labs/tests on chart.2. Assesses or performs the following:a. Vital Signsb. Weight and Height.c. Capillary or Venous blood sugar, if applicabled. Pregnancy test, if applicablee. IV start on all patients, except those without an order to do so. Utilizes anesthesia protocol/doctor's order regarding IV fluid and catheter size.f. Assess pain level3. Ensures patients receiving sedation via anesthesia provider are seen by anesthesia prior to being taken back for procedure (if applicable).4. Proficient knowledge of medications used during the admission process.Intra-Procedure:5. Reviews history & physical information and pre-op teaching form. Obtains vital signs, Aldrete score, O2 Saturation and other assessment tools according to protocol.6. Assures that the patient history, physical, ASA status and procedural consent forms are signed and on the chart.7. Assesses the patient's knowledge and understanding of the procedure and reduces patient anxiety by explaining all procedures thoroughly in a calm and reassuring manner.8. Ensures an Active "Time-out" is conducted. Verifies patient identifiers, procedure information and available equipment and supplies.9. Assists Technicians in maintaining endoscopy/ophthalmology operating rooms in a state of readiness for procedures.10. Transports patient to and from procedure with side rails up.11. Attaches appropriate monitors to the patient (cardiac, blood pressure, oxygen saturation, capnography).12. Positions the patient for procedures ensuring patient safety and comfort and privacy and maintains confidentiality of patient Protected Health Information (PHI).13. Monitors IV fluid intake according to the physician's order and patient needs.14. Administers IV sedation medications according to the physician's orders and hospital protocol.15. Assesses the patient's response to sedation including vital signs, level of consciousness and comfort level.16. Demonstrates commitment to excellence in guest relations.17. Communicates appropriate information to other members of the healthcare team in a timely manner.18. Ensures that instruments and equipment are maintained, repaired, and replaced as needed.19. Participates in overall perioperative department functions including, but not limited to the scheduling of procedures, training of new staff, maintaining appropriate statistics, and assisting with Quality Assurance projects. Also participates in the education toimprove self and others, and attending staff meetings and annual educational in-service as required.Recovery:20. Able to Care for patients in Phase I and Phase II of recovery, assessing the following: vital signs, pain level, level of consciousness per protocol.21. Carries out physician and anesthesia orders.22. Proficient knowledge of medications used in recovery.23. Records fluid intake, post-procedure blood sugar or other ordered tests, if applicable.24. Schedules follow up appointments and testing per physician's orders.25. Delivers home care instructions consistent with provider's direction.26. Discharges patient, when criteria are met, to the care of a friend, acquaintance or family member.27. Escorts patient out of facility upon discharge.Other Duties:28. Complies with attendance requirements.29. Channels patient suggestions, criticism and questions to the appropriate individuals.30. Accept and act on constructive criticisms to improve professional abilities.31. Recognizes and responds appropriately to emergency situations.32. Maintains patient medical records and recognizes legal responsibilities and functions within limitations as specified by law.33. Performs other related duties incidental to the work described herein.34. Responsible for complete and thorough performance of assigned tasks.35. Follows established rules, regulations, and policies.36. Demonstrates an awareness of safe conditions and safe acts as related to hospital employment.37. Exhibits cooperation, self-control and courtesy in relating to co-workers and job related contacts.38. Maintains adequate levels of inventory and demonstrates an economic use of equipment and supplies.39. Demonstrates respects for authority, discusses problems and follows the chain of command.40. Current licensure as a Registered Nurse in the state of Idaho. Current BLS, ACLS and PALS certification required. Current Certification for administration of Moderate Sedation required.Experience: Special training or experience in endoscopic/ ophthalmic procedures preferred. Knowledge and understanding of IV sedation medications, their actions, side effects, reversal agents, and precautions required.Language/Communication: Must have the ability to effectively read, write, and speak English.Mental Capabilities: Can frequently and consistently make sound judgments. Ability to work in a fast-paced environment.
