Practical Nurse to join our team!JOB SUMMARY: Provides direct and
indirect care services that meet the psychosocial, physical and
general aspects of care; meets the communication needs of patient
and family; provides care that reflects initiative and
responsibility indicative of professional expectations, under the
supervision of a registered nurse. Maintains regulatory agency
requirements, nursing and hospital policies, procedures and
standards. Communicates with physicians and team members about
changes in patient's clinical condition, including results of
diagnostic studies and symptomatology. Is able to respond quickly
and accurately to changes in condition or response to treatment.
Additionally, is able to perform general nursing duties in all

part-time or PRN status classification. DUTIES AND
RESPONSIBILITIES:Level I Licensed Practical Nurse:1. Is able to
perform a head-to-toe assessment on all patients and
performsreassessments as per policy. This includes, but not limited
to, geriatric and thegeneral patient population.2. Adequately
assesses and reassesses pain. Utilize appropriate pain
managementtechniques. Educates the patient and family regarding
pain management.3. Performs patient care responsibilities
considering needs specific to the standard ofcare for patient's
age.4. Is knowledgeable of medications and their correct
administration based on age of thepatient and patient's clinical
condition.5. Follows the five medication rights and reduces the
potential for medication errors.6. Correctly obtains laboratory
tests; such as Urine Pregnancy Tests, Blood Glucosetests, PT/PTT,
and others as indicated by the physician.7. Is able to interpret
results of laboratory tests accurately, taking appropriate
actionand notifying RN and Physician of test(s) results.8.
Formulates a teaching plan based on identified patient learning
needs and evaluateseffectiveness of learning; family is included in
teachings as appropriate.9. Establishes that the patient meets all
required elements for a safe discharge fromMountain View
Hospital.10. Is capable of performing treatments and providing
services to level of licensure.11. Treats patients and their
families with respect and dignity. Identifies andaddresses
psychosocial, cultural, ethnic and religious/spiritual needs of
patientsand their families.12. Interacts professionally with
patient/family and involves patient/family in formationof the plan
of care.13. Identifies and addresses psychosocial needs of patients
and family; communicateswith Social Service regarding both
in-hospital and post discharge as needed.14. Performs all aspects
of patient care in an environment that optimizes patient safetyand
reduces the likelihood of medical/health care errors.15. Carries
out medical and surgical asepsis during treatments and
specialprocedures.16. Coordinates and supervises patient care under
the direct supervision of an RN.17. Consults other departments, as
appropriate, to provide for an interdisciplinaryapproach to the
patient's needs.18. Communicates appropriately and clearly to team
leader, co-workers andphysicians.19. Meets current documentation
standards and policies.20. Appropriately responds to the patient in
an emergency of a physically distressfulsituation.21. Maintains a
good working relationship both within the department and with
otherdepartments.22. Is flexible, organized and functions well
under stressful situations.23. Manages and operates equipment
safely and correctly.24. Performs other duties as assigned within

QualificationsEducation/Certification: Graduate of an accredited
LPN program. Holds a current state LPN Licensure. Maintains a
current BLS certification. Completed hospital orientation and
annual mandatory education and training. Current IV certification
required.Experience: Six months to one year of nursing
experience.Language/Communication: Able to communicate effectively,
in English, both verbally and in writing, thus assuring effective
communication with all hospital staff, medical staff, patients and
their families by providing and maintaining open lines of
communication. Has the ability and knowledge to assist
communication for patients who speak another language other than
English.Mathematical Skills: Must be able to calculate correct dose
of medication as directed by licensure.Mental Capabilities: Must
have the ability to concentrate for long periods of time in order
to complete assigned tasks with a better than average attention
span in order to listen, read and remember verbal and written
instructions. Can understand input from the supervisor and/or
manager concerning errors and instructions.Interpersonal: Ability
to work well with others.Performance: Basic computer knowledge.
Must be able tohandle/defuse conflict issues with patients, their
families and staff.
