As a Case Manager you will be joining a collaborative team that is dedicated to serving others in need and also supporting one another. Your acute care background as a registered nurse will give you the experience you need to be highly successful in helping our patient population in impactful ways. In this role:

  • You will be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay and managing the length of stay.

  • You will ensure appropriate resource management and develop a safe and appropriate discharge plan in collaboration with the multidisciplinary team.

  • You will facilitate the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. 

  • You will lead a multidisciplinary team in the investigation, development, and implementation of the standards of care for specific patient populations and ensure successful progression and implementation of the program.

  • You will serve as liaison and subject matter resource to physicians, staff, patients, families, and professional organizations to ensure that optimal patient care is provided to patients.

  • Note: This position will be approximately 50% assisting with leadership duties and 50% working as a Case Manager within the department.

 

The largest medical facility in the region, Eastern Idaho Regional Medical Center (EIRMC) includes a full-service hospital with 318 patient beds. It serves as the region’s healthcare hub, serving Southeast Idaho, Western Wyoming, Yellowstone National Park and parts of Montana.  EIRMC is home to a Level II Trauma Center, a Level 1 ICU (of only two in Idaho), and the only Burn Center in the state of Idaho. Our ER is the most experienced in the region. The hospital's other specialty services include the region's only Level III Neonatal Intensive Care Unit capable of caring for babies born as early as 22 weeks gestation; the region’s only Pediatric Intensive Unit, the largest heart program in the region, a Behavioral Health Center which is a74-bed Joint Commission accredited psychiatric hospital  and the region’s only Primary Stroke Center.

Qualifications

  • Current and valid license to practice as a Registered Nurse (ADN or BSN) in the state of Idaho; Bachelor’s Degree is preferred.
  • One year of hospital case management experience required.
  • At least 3-5 years of acute care nursing experience required.
  • Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement preferred.
  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification
  • Professional certification as a Case Manager is preferred.
